A toddler died after swallowing a plastic googly eye that was stuck in his physique for 14 months.

Hospital scans failed to select up the item that was lodged in two-year-old's Kazarie Dwaah-Lyder's body from February 2022.

The toddler from North Woolwich, London, died at Nice Ormond Road Hospital on April 27 2023 because of bleeding from a gap in his oesophagus.

His medical explanation for dying was given as 1a upper gastrointestinal bleed and haemorrhagic shock, 1b oesophageal tear extending to the aorta, and 1c overseas body in oesophagus.

A publish on Kazarie's GoFundMe reads: 'We're deeply saddened to inform you that our beloved youngster Kazarie Kwaku Dwaah-Lyder; aged 2, handed away on Thursday 27th April 2023 at 4:01am, at Great Ormand Road resulting from unexpected circumstances and extreme problems. (A gap found in his oesophagus).

'Kazarie was a tremendous, sensible, humorous and loving little boy who had an previous soul, he touched the hearts of many, and we need to give him the farewell he deserves.

'We understand that this can be a troublesome time for everyone, but we kindly ask for any quantity you can give, regardless of how small, to help us honour his mum and older brother in addition to prolonged household.'

Speaking concerning the case, Coroner Mary Hassell advised the courtroom: 'I used to be informed that there is a lack of national steerage for such a state of affairs.

'I respect that there are multiple issues in planning investigations, such because the dangers related to CT scanning and the dangers related to the administration of a common anaesthetic.

'It appears that evidently the matter would profit from consideration at a nationwide degree.'

Ms Hassell's report has been despatched to the British Affiliation of Paediatric Surgeons, the Royal School of Paediatrics and Youngster Well being, and the Royal School of Radiologists.

The BAPS confirmed it had acquired the coroner's report, and can be responding earlier than the April 8 deadline given by the coroner.

A spokesperson added: 'We have been saddened to hear about this case and our thoughts exit to Kazarie's household.'

An RCPCH spokesperson stated: 'That is a completely tragic case and our thoughts go out to Kazarie's household.

'I can affirm that RCPCH has acquired the coroner's report and might be responding inside the timeframe said.'
